糖尿病脑梗死及非糖尿病高血压脑梗死患者的临床研究 被引量:17

摘要 目的探讨糖尿病脑梗死与非糖尿病高血压脑梗死患者的临床特点。方法糖尿病并发脑梗死患者85例,其中糖尿病高血压脑梗死组57例,糖尿病正常血压脑梗死组28例;同期住院的无糖尿病高血压脑梗死组40例;正常对照组40例。观察血压、血糖、血脂、肾功能及预后。结果高血压脑梗死组的舒张压(DBP)及平均动脉压(MAP)明显高于糖尿病脑梗死组(P<0.05);3组脑梗死均存在明显的脂代谢紊乱,BUN和Cr水平均明显高于正常对照组,尤以糖尿病高血压脑梗死组增高更明显(P<0.05);糖尿病高血压脑梗死组恶化病死率最高(P<0.05)。结论脑梗死患者均存在脂代谢异常及肾功能紊乱,糖尿病高血压脑梗死组预后最差。 Objective To investigate the clinical characteristics on diabetic cerebral infarction and hypertensive cerebral infarction without diabetes mellitus.Methods Eighty-five cases of diabetic cerebral infarction(57 cases with hypertension and 28 cases without hypertension) and 40 cases of hypertensive infarction without diabetes were studied retrospectively.The blood pressure,blood lipoid,kidney function and prognosis were observed respectively.Results Diastolic pressure(DBP) and mean artery pressure(MAP) in group of hypertensive cerebral infarction were highist than those in groups of diabetic cerebral infarction(P〈0.05).There was a significant disturbance on blood lipoid in three groups of cerebral infarction,and the levels of their BUN and Cr were significantly higher than those in group of control,especially diabetic infarction with hypertension.There was the highist mortality in group of diabetic cerebral infarction with hypertension.Conclusion The disturbances of lipometabolism and of kidney function all exist in the patients of cerebral infarction with and without diabetes mellitus.The patients of diabetic cerebral infarction with hypertension have the worst prognosis.
