摘要
目的探讨老年2型糖尿病伴高脂血症患者对尿白蛋白水平的影响及相关因素分析。方法检测单纯高脂血症患者1 464例(HL组)、单纯2型糖尿病患者469例(T2DM组)、2型糖尿病伴高脂血症患者703例(T2DML组)和正常对照人群1 301例(C组)的空腹血糖(FBG)和餐后2 h血糖(2 hBG)、血脂和尿蛋白肌酐比值测定(ACR)。结果 T2DML组ACR明显高于HL组及T2DM组,此3组ACR均明显高于C组,差异有统计学意义(P<0.05)。Spearman相关分析显示:ACR与FBG(r=0.177,P<0.05)、2 hBG(r=0.167,P<0.05)、体质指数(BMI)(r=0.087,P<0.05)、收缩压(SBP)(r=0.212,P<0.05)、舒张压(DBP)(r=0.133,P<0.05)、总胆固醇(TC)(r=0.084,P<0.05)、三酰甘油(TG)(r=0.133,P<0.05)呈正相关,多元逐步回归分析显示ACR与年龄、FBG、SBP、性别呈独立正相关(P<0.05)。结论单纯高脂血症及单纯糖尿病引起尿蛋白排泄率增高,2型糖尿病伴高脂血症尿蛋白排泄增高更明显,脂代谢紊乱可能加重尿蛋白排泄。
Objective To investigate the effect of hyperlipidemia on urine album in elderly patients with type 2 diabetes mellitus complicated by hyperlipidemia and to analyze its relative risk factors. Methods Totally 1 464 patients with hyperlipidemia（HL group）,469 patients with type 2 diabetes mellitus（T2DM group）,703 patients with both hyperlipidemia and 2 type diabetes mellitus（T2DML group） and 1 301 normal control subjects（NC group） were enrolled.Fasting blood glucose（FBG）,2-hour postprandial blood glucose（2hPG）,blood lipid,and albumin-to-creatinine ratio（ACR） were measured for all the subjects. Results The ACR of the T2DML group was significantly higher than that of the HL and T2DM groups,and the ACRs of this three groups were all significantly higher than that in the NC group（P0.05）.Spearman correlation analysis showed that ACR was positively correlated to FBG（r=0.177,P0.05）,2h-BG（r=0.167,P0.05）,BMI（r=0.087,P0.05）,systolic blood pressure（SBP）（r=0.212,P0.05）,diastolic blood pressure（r=0.133,P0.05）,TC（r=0.084,P0.05）,and TG（r=0.133,P0.05）.Multiple stepwise regression analysis showed that ACR was independently positively correlated with the age,FBG,SBP,and sex（P0.05）. Conclusion Urine albumin excretion rate is increased in simple hyperlipidemia or type 2 diabetes mellitus,and it is higher diabetes mellitus complicated by hyperlipidemia.Metabolite turbulence of blood lipid can aggravate urine albumin excretion rate.
