单心动周期实时三维超声心动图评价左心室收缩功能 被引量:4

摘要 目的探讨单心动周期实时三维超声心动图(sRT-3DE)评价左心室收缩功能的可行性及临床价值。方法应用sRT-3DE与多心动周期实时三维超声心动图(mRT-3DE)测量50例健康志愿者(正常组)和27例左心室扩大者(病例组)的左心室舒张末容积(LVEDV)、收缩末容积(LVESV)、每博量(SV)及射血分数(EF),分别比较正常组和病例组sRT-3DE和mRT-3DE左室收缩功能指标的测量值;分析上述两种方法的相关性。结果①所有受试者均获得了满意的实时三维超声图像。正常组及病例组sRT-3DE与mRT-3DE法所测LVEDV、LVESV、SV、EF值差异均无统计学意义(P均>0.05)。②正常组和病例组sRT-3DE及mRT-3DE法测值相关性良好。正常组r分别为0.805、0.658、0.843、0.927(P均<0.05),病例组r分别为0.955、0.873、0.917、0.978(P均<0.05)。③sRT-3DE法观察者内及观察者间测值均具有良好的重复性,变异系数分别为7.27%、8.94%、10.42%、6.51%和7.44%、10.66%、9.39%、7.07%。结论 sRT-3DE图像无拼接伪像,可实时、准确评价左心室收缩功能,尤其适用于左心室扩大者。
