胆总管探查23例临床分析

Clinical analysis of 23 Cases of exploring common bile duct

目的:探讨不同胆总管探查术式和引流方式的临床效果。方法:回顾性分析我科23例胆总管探查的临床资料,其中12例行T管引流术,6例行胆肠内引流术,2例行经胆囊管胆总管探查,3例行I期缝合术。结果:T管引流者术后胆管出血l例,胆道残留结石l例I,期缝合者出现漏胆1例,胆总管穿孔性损伤1例,均治愈,术后出现返流性胆管炎3例。结论:掌握胆总管探查的适应证,选择合理的术式及引流方式,做好并发症的预防和处理,是手术成功的关键。

Objective：To investigate the clinical effectiveness of different operation and drainage ways for exploring common bile duct.Methods：The clinical data of 23 cases of Exploring common bile duct were retrospectively analyzed.In those cases,12 received T-tube drainage,6 received biliary-enterostomy,2 received exploring through cystic duct,3 received primary suture.Results：1 had postoperation hemorrhage and 1 had residual stones in common bile duct by T-tube drainage,1 had biliary leakage by primary suture,1 had intraoperative perforation of common bile duct.All the compilications were cured,3 had contraflow cholangitis.Conclusion：The key to success of exploring common bile duct was to grasp operation indication of exploring common bile duct,choose right ways of operation and prevent complications.
