2145例胸部创伤的诊治体会

Diagnosis and therapy of 2145 cases chest traumatic patients

目的:探讨胸部创伤患者的临床诊治措施,以提高救治成功率。方法:回顾胸部创伤患者2145例,采用病因及伤情判断、液体复苏、早期手术干预及其它对症支持治疗等临床诊治措施。结果:2145例患者中轻度1103例(51.42%),中度738例(34.41%),重度303例(14.17%),重度中死亡20例(0.9%),成功率99.1%,其中256例患者进行各种手术治疗。结论:胸部创伤将会导致呼吸、循环功能障碍,其诊治重点是动态观察病情以明确诊断,采取手术和液体复苏及其它对症治疗,改善组织细胞的血供和氧合,以降低早期病死率并防止全身性炎症反应综合征(SIRS)、多器官功能障碍(MODS)、多器官衰竭(MOF)等并发症的发生。

Objective：To explore the diagnosis and therapy of chest traumatic patients for better managements and prognosis.Methods：2145 cases chest trunmatic patients were enrolled in our department.After diagnosis according to clinical manifestation and injury score,the patients were treated with fluid resuscitation,early operation and other therapies.Results：all of 2145 chest traumatic patients,1103 cases（51.42%） were mild degree,738 cases（34.41%） were moderate degree,303（14.17%） were severe degree.20 of 303 cases severe chest traumatic were dead,the mortality was 0.9%.256 cases（11.93%） accepted the operations.Conclusion：chest traumatic is common and critical.The key managements include dynamic observation,surgical operation,fluid resuscitation and other supportive therapies,which are beneficial to improve the blood and oxygen supply,decrease early mortality and eventually prevent various severe complications such as SIRS,MODS and MOF.
