多原发癌发病的遗传因素——附22例临床报道

Genetic factors of the multiple primary carcinoma——A clinical report of 22 cases

目的探讨多原发癌(multiple primary carcinoma,MPC)发病的遗传学因素。方法总结22例多MPC的临床特点,结合文献复习MPC发生的遗传学基础。结果本组经病理证实的MPC22例,其中男性14例,女性8例。有家族史8例,占36.4%。首发癌前3位为结直肠癌6例,胃癌4例,乳腺癌4例;第二原发癌前3位为结直肠癌7例,肺癌5例,食管癌3例。同时性癌5例,异时性癌17例。第一和二原发癌发生相距中位时间38月(0～116月)。符合文献报道范畴和当地肿瘤疾病谱。结论鉴于MPC发病的遗传学因素,对有明显家族史的癌症患者,随访时要特别注意MPC的发生。

Objective： To explore the genetic factors of the multiple primary carcinoma（MPC）.Methods：Review the genetic foundation in MPC,by summariising the documents and clinical features of 22 cases MPC.Results：This group has 22 cases MPC with pathological diagnosis,including male 14,female 8 cases.There is 8 cases（36%） have the family history of carcinoma.The sequence of the first primary cancer is colorectal cancer（6 cases）,gastric cancer（4 cases）,breast cancer（4 cases）;the sequence of the second primary cancer is colorectal cancer（7 cases）,lung cancer（5 cases）,esophageal cancer（3 cases）.There are 5 cases synchronous cancer and 17cases of metachronous cancer.It is 36 months（0～116 months） that the median time of occurrence of the first and the second primary cancer.Consist with the report and local tumor incidence rate.Conclusion：As the genetic factors of multiple primary cancer,pay attention to the patients with a family history of cancer for the occurrence of MPC while following-up.
