胰腺实性假乳头状瘤5例临床病理分析

Clinicopathological Analysis of 5 Cases of Solid Pseudopapillary Tumor of Pancreas and Review of Literature

目的探讨胰腺实性假乳头状瘤(SPTP)临床病理学特征及诊治要点。方法对2005至2010年我院收治的5例SPTP的临床病理资料、免疫组化表型及治疗进行回顾性分析。结果 5例SPTP患者男女比例1:4,均获手术治疗,疗效满意。该肿瘤呈囊实性,包膜完整,肿瘤细胞组织学形态较一致,并形成实性及假乳头状结构。免疫组化结果:肿瘤细胞表达V im、Syn、CD56、CD10、AAT、AACT;不表达CgA。结论 SPTP是一种罕见的多发于年轻女性的低度恶性肿瘤,其无特异性临床表现,但组织学图像有特征性的假乳头结构,免疫组化结果对其诊断有重要价值,手术切除为其主要治疗方式,预后良好。

Objective To study the clinical and pathologic features and main points of diagnosis and treatment of solid pseudopapillary tumor of the pancreas（SPTP）.Methods The clinical histopathological data,immunohistochemical features and treatment of 5 cases of SPTP in this hospital from 2005 to 2010 were analyzed.Results The ratio of female to male was 4：1,and all cases were suffered operation.The satisfied results were obtained.SPTP was solid-cystic and grossly appeared encapsulated.The histopathological features of the tumor cells were uniform with typical pseudopapillary pattern.Immunohistochemical results showed the tumor cells were positive for Vim,Syn,CD56,CD10,AAT,AACT,but negative for CgA.Conclusion SPTP is an uncommon and low grade malignant neoplasm,which is frequently found in young female.SPTP has no specific symptoms,but has typically histological character which is the pseudopapillary structure.Immunohistochemical result is important for the diagnosis of SPTP.The surgical excision is the main treatment method and has excellent prognosis.
