白癜风患者LFA-1、ICAM-1的检测及其与移植疗效的相关性研究

Relationship Between the Efficacy of Autologous Epidermal Grafting and the Levels of Lymphocyte Function Associated Antigen-1,Intercellular Adhesion Molecule-1 in Vitiligo

目的探讨白癜风自体表皮移植疗效与LFA-1、ICAM-1的相关性。方法对20例稳定期白癜风患者进行负压吸疱移植治疗并收集白斑及非白斑区的疱液,用ELISA方法测定组织液中LFA-1、ICAM-1的水平。移植后1个月观察疗效。结果 20例白癜风患者局部白斑区与非白斑区组织液ICAM-1、LFA-1水平相比,具有统计学差异。14例寻常型白癜风患者局部白斑与非白斑区组织液ICAM-1、LFA-1水平比较具有统计学差异。6例节段型白癜风患者白斑与非白斑区组织液ICAM-1、LFA-1水平比较无统计学差异。移植失败组白斑区ICAM-1的浓度明显高于成功组(P<0.05),非白斑区ICAM-1的浓度比较则无统计学意义。两组之间LFA-1的浓度比较无统计学意义。结论 LFA-1及其配体ICAM-1的异常表达对白癜风患者皮损区的微环境改变有一定的影响,可能与自体表皮移植疗效相关。

Objective To investigate the relationship between the efficacy of autologous epidermal grafting and the levels of lymphocyte function associated antigen（LFA）-1,intercellular adhesion molecule（ICAM）-1 in vitiligo.Methods 20 patients with stable vitiligo receiving autologous epidermal grafting were included in this study.Suction blister fluid was collected from vitiligous lesions and normal skin in these patients.Enzyme-linked immunosorbent assay（ELISA） was used to detect the levels of LFA-1 and ICAM-1 in suction blister fluid.Clinical efficacy was evaluated through a month follow-up after grafting.Results Among these 20 patients,the expression of ICAM-1 and LFA-1 in suction blister fluid was significantly higher in vitiligous lesions than that in normal skin.The expression of ICAM-1 and LFA-1 in suction blister fluid was significantly higher in vitiligous lesions than that in normal skin in 14 patients with common type vitiligo,while there was no significant difference in six patients with segmental type vitiligo.We found the expression of ICAM-1 in suction blister fluid of vitiligous lesion was significantly higher in those patients who responded poorly than in those patients who responded successfully,while there was no significant difference in normal skin.Meanwhile,there was no significant difference of the expression of LFA-1 in suction blister fluid in the two groups.Conclusion The abnormal expression of LFA-1 and its ligand ICAM-1 had some effects on the microenvironment in vitiligo patients,which might have some relationship with the efficacy of autologous epidermal grafting.
