摘要 目的探讨彩色多普勒超声诊断卵巢肿瘤蒂扭转的价值。方法回顾性分析27例经手术和病理证实的卵巢肿瘤蒂扭转患者的超声资料,并根据血管蒂的彩色多普勒表现将其分为3型。随访27例患者的手术及病理结果,与超声表现进行对比分析。结果本组23例患者于卵巢肿瘤旁可探及扭转血管蒂,呈"双肿块征"。其彩色多普勒表现为Ⅰ型4例、Ⅱ型5例、Ⅲ型14例。Ⅰ型患者手术切除肿瘤及附件送病理检查均无坏死;Ⅱ、Ⅲ型患者术后病理诊断均有出血性梗死;彩色多普勒超声诊断卵巢肿瘤蒂扭转的符合率85.2%(23/27)。结论彩色多普勒超声对诊断卵巢肿瘤蒂扭转具有重要价值。 Objective To investigate the value of color Doppler ultrasound in diagnosis of ovarian tumors with pedicle torsion. Methods Ultrasonic appearance of 27 patients with torsion of ovarian tumors pedicle confirmed by surgery and pathology were analyzed retrospectively, they were divided into three types according to the manifestation of pedicle torsion. The surgical and pathological results of 27 patients were followed up, then compared with ultrasonic appearance. Results The torsion vascular pedicle of ovarian tumor with a "two - clump" sign could be detected in 23 patients, their color Doppler appearance included type I (4 cases ), type Ⅱ (5 cases ), and type Ⅱ(14 cases). No tissue necrosis was detected by pathological examination in the samples of tumor and the appendage in type Ⅰ , while hemorrhagic infarct were found in type Ⅱ and type Ⅲ, the diagnostic accuracy rate of color Doppler ultrasound for torsion of ovarian tumors pedicle was 85.2% ( 23/27 ). Conclusion Color Doppler ultrasound has important value in diagnosis of ovarian tumors with pedicle torsion.
