ERα基因多态性与更年期焦虑抑郁发生的相关性 被引量:9

Correlation between polymorphism of estrogen receptor α gene and anxiety and depression during menopause

摘要 目的:探索ERa基因多态性与更年期焦虑抑郁发生的相关性。方法:收集更年期焦虑抑郁患者78例(焦虑抑郁组)和无焦虑抑郁症状的更年期妇女72例(对照组),采用限制性片段酶切基因多态性分析方法(RFLP)分析所有样本的PvuⅡ、XbaⅠ基因多态性,采用Greene更年期症状量表分析临床症状群的特征,采用心理症状评分(HAMD、HAMA)评定样本的抑郁焦虑程度,采用简易精神智力评定量表(MMSE)评定认知功能。结果:①焦虑抑郁组的MMSE评分和E2、P水平较对照组下降(P<0.01)。焦虑抑郁组Greene评分的焦虑症状群、抑郁症状群及躯体症状群与对照组相比明显增高,血管舒缩症状群明显降低(P<0.01)。②ERα基因的PvuⅡ和XbaⅠ的等位基因频率和基因型在两组中无统计学差异(P>0.05),Logistics回归分析显示,ERα基因型与各症状群无相关性。ERα基因型与MMSE的评分呈现负相关(r=0.108,P=0.033)。结论:ERα基因的PvuⅡ和XbaⅠ的等位基因与焦虑抑郁障碍的发生无相关性,ERαPvuⅡ的Pp基因型与焦虑抑郁障碍的认知功能下降相关。 Objective:To explore the correlation between polymorphism of estrogen receptor α gene and anxiety and depression during menopause. Methods:78 menopausal women with anxiety and depression(anxiety-depression group) and 72 menopausal women without anxiety and depression(control group) were selected,RFLP was used to analyze the polymorphisms of Pvu Ⅱ and Xba Ⅰ,Greene-Climacteric-Scale was used to analyze the characteristics of clinical symptoms,Psychological symptom scores(HAMD,HAMA) were used to assess the degrees of anxiety and depression,MMSE was used to assess the cognitive function. Results:The MMSE score,estrogen and progesterone levels in anxiety-depression group were significantly lower than those in control group(P〈0.01).The scores of anxiety symptoms,depression symptoms and somatic symptoms in anxiety-depression group assessed by Greene-Climacteric-Scale were significantly higher than those in control group,the score of vasomotor symptoms decreased significantly(P〈0.01).There was no significant difference in allele frequency and genotype of estrogen receptor α gene Pvu Ⅱ and Xba Ⅰ between the two groups(P〉0.05).Logistic analysis showed that there was no correlation between genotype of estrogen receptor α and various symptoms;there was a negative correlation between genotype of estrogen receptor α and MMSE score(γ=0.108,P=0.033). Conclusion:There is no correlation between Pvu Ⅱ and Xba Ⅰ allele of estrogen receptor α and occurrence of anxiety-depression disorder,Pp genotype of estrogen receptor α gene Pvu Ⅱ is related to the decrease of cognitive function of patients with anxiety-depression disorder.
