胰管结石手术治疗11例临床分析

Clinical analysis of surgical treatment of 11 patients with pancreatic duct stones

目的总结胰管结石手术治疗的经验。方法对2003～2009年手术治疗的11例胰管结石患者临床资料进行回顾性分析。结果 11例结石均成功取出。其中,8例行胰管切开取石、胰管-空肠Roux-en-Y吻合术,2例行胰十二指肠切除,1例行胰体尾切除加脾切除术、胰断端套入空肠端Roux-en-Y吻合术。术后临床症状均有不同程度的缓解。结论外科手术是治疗胰管结石的有效方法;胰管切开取石加胰管-空肠Roux-en-Y吻合术(改良Puestow)是治疗胰管结石的主要术式。

Objective To summarize the experience in surgical treatment of pancreatic duct stones.Methods Data of 11 patients who underwent surgical operation for pancreatic duct stones between 2003 and 2009 were retrospectively analyzed.Results Pancreatic duct stones were taken out successfully in 11 cases,of whom 8 cases were treated with pancreatolithotomy and Roux-en-Y side-to-side pancreaticojejunostomy,2 cases underwent pancreaticoduodenectomy,and 1 case underwent distal pancreatectomy and splenectomy Roux-en-Y end-to-end pancreaticojejunostomy.Remission of clinical symptoms was achieved in all cases postoperatively.Conclusion Surgical operation is effective in treatment of patients with pancreatic duct stones.Pancreatolithotomy plus Roux-en-Y pancreaticojejunostomy（modified Puestow） is a main method in the patients with pancreatic duct stones.
