Velocity vector imaging in quantitative analysis of left ventricular strain and strain rate of coronary heart disease before and after percutaneous transluminal coronary angioplasty plus stent implantation therapy

摘要 目的探讨VVI在早期诊断冠心病(CHD)和评价经皮冠状动脉成形术(PTCA)+支架植入术疗效的意义。方法利用VVI软件分析CHD组(n=21)支架植入术前1天、术后7天左前降支(LAD)供血的9个节段的收缩期峰值速度(Vs)、应变(ε)、应变率(SRs、SRe),并与对照组(23名正常人)比较。结果 CHD组术前LAD供血的9节段Vs、ε、SRs、SRe明显低于对照组相应节段(P均<0.05);PTCA+支架植入术后7天,大部分节段ε较术前均显著提高(P均<0.05),但与对照组仍有差异(P均<0.05)。结论 VVI技术可无创定量评价PTCA术后早期疗效,对于评价CHD患者PTCA治疗后的恢复情况有重要意义。 Objective To assess the significance of VVI in early diagnosis of coronary heart disease(CHD) and evaluation of therapeutic effect of percutaneous transluminal coronary angioplasty(PTCA) plus stent implantation.Methods VVI software was used to analyze systolic peak velocity(Vs),strain(ε),systolic and early-diastolic strain rate(SRs,SRe) of 9 segments supplied by left anterior descending(LAD) in CHD group(n=21) before and 7 days after PTCA+stent therapy.Then the results were compared with those of 23 normal controls(control group).Results Vs,ε,SRs and SRe of 9 segments supplied by LAD were obviously lower in CHD group than in control group before therapy(all P0.05).The ε in most segments improved 7 days after PTCA+stent therapy(all P0.05),but still lower than that in control group(all P0.05).Conclusion VVI can quantitatively analyze the left ventricular regional myocardial systolic functional changes in 7 days after PTCA+stent therapy.It has important significance for judging prognosis of CHD and evaluating therapeutic efficacy of PTCA.
