心律平的儿童药代动力学和临床研究

Pharmacokinetic and Clinical Studies of Propafenone in Children

本文探讨了静脉注射心律平在儿童体内的药代动力学规律,其血药浓度个体差异较大,药一时曲线与文献报道基本一致。T(1/2)β为2.65小时。5例静注心律平(3mg/kg)均即刻出现疗效,即刻达最高有效血浓度,15分钟后明显下降。说明心律平确为一高效、速效且安全的抗心律失常药,可作为控制快速型心律失常的有效药物。13例口服,吸收良好,连续服用可达稳态血浓度,适用于治疗各类早搏和心动过速发作后的维持治疗。心律平的有效血浓度范围极广,剂量一血药浓度关系的个体差异性很大,治疗上应力求做到给药方案个体化。

The pharmacokinesis of propafenone i.v.in children was studied.It showed agreat interindividual variability in propafenone pharmacokinesis.The time-effect curve in this study is similar to those in the literature reported,t_(1/2)β being 2.65 hr.The peak effective plasma concentration was achieved immediately after i.v.admini-stration of propasenone in five cases and decreased significantly in 15 minutes.It isdemonstrated that propafenone is a safe,rapid and highly effective antiarrhythmicfor controlling tachycardiac arrhythmias.Oral administration of this drug in 13cases showed that it was well absorbed and a steady plasma concentration mightbe achieved with continuous administration.Thus it is a good drug for treating prematurebeats and for maintenance after tachycardiac episodes.The effective plasma concen-tration of propafenone is extensive.It has a significant interindividual variabilityin dosage-concentration relations.The protocol of treatment with propafenone shouldbe individualized.
