Fistula of the First Branchial Cleft,Report of 8 Cases

本文报告8例第一鳃裂瘘管,详细讨论了胚胎发生学、分型、诊断及治疗。认为Work 两型分法对手术治疗有指导意义。提出下颌角附近有持续性瘘管,特别是伴有耳流脓而听力及鼓膜正常者,首先考虑为本病。手术治疗最好行类似腮腺手术的切口,术中应注意保护面神经和腮腺。防止复发的关键是彻底切除瘘管组织。

Eight cases of fistula of the first branchial cleft were reported,and the em-bryopathology,classification diagnosis and treatment of the disease discussed.The cla-ssification of two types was practically helpful in directing the operation.It wassuggested that patients with persistant fistula near the angle of jaw,particularlywith pus draining out from the external acoustic meatus but with normal hearing andtympanic membrane,should first be thought of this disease.The incision like that ofparotidectomy was preferable.In the operation the facial nerve and the parotidshould be taken care from being injured.The key against recurrence was a completeresection of the fistula.
