摘要
慢性广泛性疼痛是分布广泛,迁延不愈,机制未明的一类病理性反应,常见于纤维肌痛。目前针对慢性广泛性疼痛存在多种假设。中枢水平的改变主要表现在脊髓水平与脊髓上水平。脊髓上水平的痛觉下行抑制性通路失敏与痛觉下行易化性通路兴奋,在脊髓水平上表现为相应的改变,导致中枢致敏与痛觉过敏。外周水平上,酸敏感离子通道3(ASIC3)的激活是慢性广泛性疼痛发生的必要条件,神经营养因子3能预防痛觉过敏的发生。
Objective: Chronic widespread pain as one of the conditions in fibromyalgia is characterized by generalized and long-lasting pain.The pathophysiology of chronic widespread pain is still unclear but a number of hypotheses have been proposed.Central changes show on spinal cord level and supraspinal level.Increases in excitatory and decreases in inhibitory modulation pathways were observed in supraspinal level.These alterations drive changes in the spinal cord to result in central sensitization and hyperalgesia.Peripherally,activation of acid-sensing ion channel 3(ASIC3) is necessary for development of widespread muscle pain,and neurotrophin-3 prevents development of hyperalgesia.
