通络脉合剂治疗糖尿病足的疗效观察

Curative effect of Tongluomai Heji for diabetes foot

目的观察通络脉合剂治疗糖尿病足热毒伤阴证的疗效。方法采用随机对照研究的方法,将120例糖尿病足证属热毒伤阴患者分为2组,治疗组60例,常规应用扩张血管药物、胰岛素加口服通络脉合剂,每日2次,每次100 mL,早晚饭后温服;对照组60例,常规应用扩张血管药物、胰岛素加口服四妙勇安汤,每日2次,每次100 mL,早晚饭后温服;2个月为1个疗程,治疗1个疗程后观察疗效。结果治疗组临床痊愈36例,好转21例,无效3例,总有效率95.0%。对照组临床痊愈22例,好转26例,无效12例,总有效率80.0%,治疗组总有效率明显高于对照组(P<0.05);治疗组在肢体皮肤发凉、肢体麻木、间歇性跛行、深浅感觉障碍及溃疡愈合方面总有效率高于对照组,差别有统计学意义(P<0.05);治疗组治疗后全血黏度、血浆黏度、红细胞比积、血沉、纤维蛋白原低于治疗前及对照组治疗后,差别均有统计学意义(P<0.05);对照组治疗前后各指标间比较,差别无统计学意义(P>0.05)。结论通络脉合剂为主的治疗方法效果优于四妙勇安汤为主的治疗方法,通络脉合剂是治疗糖尿病足的安全药物。

Objective To observe the curing effect of Tongluomai Heji for syndrome of yin injured by pyretic toxicity with diabetes foot（necrosis）.Methods One hundred and twenty cases of diabetes foot syndrome from yin injured by pyretic toxicity were divided into treatment group and control group with randomised control trial.The treatment group were conventionally treated with vasodilators,insulin and oral Tongluomai Heji,twice a day,each 100 mL,after breakfast and supper.The control group were conventionally treated with vasodilators,insulin and oral Simiao Yong′an Tang,twice a day,each 100 mL,after breakfast and supper.The patients all were treated for two months as one course of treatment.Then the curative effect were observed.Results In treatment group,thirty-six cases were cured clinically,twenty-one cases were improved,and three cases were ineffective;The total effective rate was 95.0%.In control group,twenty-two cases were cured clinically,twenty-six cases were improved,and twelve cases were ineffective,the total effective rate was 80.0%.By comparing the total effective rate,that in the treatment group is obviously higher than that in control group（P〈0.05）.The total effective rates in improving chilly limbs skin,limbs numbness,intermittent claudication,superficial sensation and deep sensation disorder and healing of ulcer in treatment group were higher than those in control group（P〈0.05）.The whole blood viscosity,blood plasm viscosity,packed cell volume,hematocrit,blood sedimentation,fiber proteinogen,were lower than before treating and lower than those in control group after treating（P〈0.05）.The comparison of the indexes in control group before and after treatment had no statistical significance（P〉0.05）.Conclusion The curing effect of the therapeutics with Tongluomai Heji is more effective than the therapeutics with Simiao Yong′an Tang.Tongluomai Heji is an effective and safe remedy for diabetes foot.
