HLA-Ⅰ、CD8和CD4在宫颈不同病变组织中的表达及意义

Expressions and Significance of HLA Class Ⅰ Antigens,CD8 and CD4 in Various Cervical Diseases

目的检测人类白细胞抗原1(HLA-Ⅰ)类抗原、CD8和CD4分子在慢性宫颈炎、宫颈上皮内瘤样变(CIN)、宫颈癌和癌旁组织中的表达,探讨其表达与宫颈不同病变的关系。方法宫颈癌、CIN、慢性宫颈炎和癌旁组织标本各20份,采用免疫组织化学SP法检测HLA-Ⅰ类抗原、CD8和CD4分子的表达。结果在宫颈癌、CIN、慢性宫颈炎和癌旁组织中,HLA-Ⅰ类抗原的阳性表达率分别为40%、95%、100%和100%,宫颈癌组的表达率明显降低(P<0.01);CD8分子的阳性表达率分别为35%、95%、100%和100%,宫颈癌组的表达率明显降低(P<0.01);CD4分子的阳性表达率分别为45%、80%、100%和100%,宫颈癌组的表达率亦明显降低(P<0.01)。HLA-Ⅰ类抗原和CD8分子的表达呈正相关(r=0.913,P<0.001)。结论 HLA-Ⅰ类抗原、CD8和CD4分子在宫颈上皮内瘤变和宫颈癌组织中表达减少及无表达,3者在宫颈病变程度由低到高的发生发展过程中可能起着重要的保护作用。

Objective To examine the expressions of HLA class Ⅰantigen, CD8 and CIM in various cervical diseases and investigate their association with cervical cancer. Methods The expressions of HLA class Ⅰ antigen, CD8 and CD4 in patients with cervical cancer,cervical intraepithelial neoplasia（CIN） ,chronic cervicitis and cancer adjacent tissues were detected by SP immunohistochemistry. Results The positive expression rates of HLA class Ⅰ antigen in cervical cancer, CIN, chronic cervicitis and cancer adjacent tissues were 40% ,95%, 100.0% and 100.0% ,respectively, the positive expression rates of HLA class I antigen in patients with cervical cancer were significantly lower than those in others（P 〈0. 01 ） and the positive expression rates of CD8 were 35% ,95%, 100% and 100.0%, respectively, CD8 in patients with cervical cancer were significantly lower than those in others （ P 〈 0.01 ）, The positive expression rates of CD4 were 45%, 80%. 100% and 100%, respectively. The positive rates of CD4 in patients with cervical cancer were significantly lower than those in others（P 〈0.01）. A positive correlation was found between HLA class I antigen and CD8 expression[ Spearmang correlation（r =0. 913 ,P 〈0. 001 ） ]. Conclusion The expressions of HLA class Ⅰ antigen, CD8 and CD4 are down-regulated or absent in CIN and cervical cancer,and they may play important roles in the development and progression of CIN and cervical cancer.
