关节镜下膝关节前交叉韧带断裂同种异体肌腱重建术的临床研究

A clinical study on reconstruction of anterior cruciate ligament with homologous under arthroscopy

[目的]探讨膝关节镜下采用同种异体肌腱重建前交叉韧带的方法和效果。[方法]回顾性分析2007年3月～2009年4月18例关节镜下应用同种异体肌腱重建前交叉韧带的情况,应用山西骨组织库提供的同种异体肌腱进行前交叉韧带重建,观察并记录治疗前后的症状、体征变化,采用Lysholm关节评分评价疗效。[结果]全部病例均获得随访,平均随访25个月,Lysholm关节评分由术前(41±1.1)分提高到术后(92±2.6)分(P<0.01),在统计学上有显著差异。所有病例均无膝前区疼痛,2例体检仍出现阳性体征,关节功能明显改善,症状基本消失。[结论]关节镜下同种异体肌腱重建前交叉韧带创伤小,稳定性好,并发症少,疗效优良。可避免自体移植物取材后造成的并发症。

[Objective]To explore the clinical effect of anterior cruciate ligament with homologous ligaments under arthroscopy. [Method]Eighteen patients with anterior cruciate ligament injury were reviewed retrospectively.All were reconstructed by homologous ligaments under arthroscopy.The Lysholm score were recorded before and after operation. [Result]All the patients were followed up for average of 25 months,statistically significant differences were seen in Lysholm score between preoperatively and postoperatively（average 41±1.1 preoperatively and 92±2.6 postoperatively）（P0.01）.The difference was significant.Symptoms were relieved and the function of knee joint was improved.The unstable symptom of two knees was still anterior drawer positive. [Conclusion]Reconstruction of anterior cruciate ligament with homologous ligaments is effective for avoiding complications of autografts.The operation under arthroscopy has advantages of less injury,rapid recovery and satisfactory effect.
