摘要
目的探讨电化学发光免疫技术(ECLIA)定量检测乙型肝炎病毒血清标志物(HBV-Markers,HBV-M)与酶联免疫吸附试验(ELISA)定性检测HBV-M的优、缺点及临床意义。方法采用ECLIA法和ELISA法同时测定80例HBV感染者和30例体检健康者HBV-M五项指标(HBsAg、抗-HBs、HBeAg、抗-HBe和抗-HBc),并对两种方法检测结果进行比较。结果两种方法测定HBsAg、抗-HBs、HBeAg、抗-HBe和抗-HBc结果符合率分别为94.54%、95.45%9、4.54%、80.00%、95.45%。经相关分析:HBsAg、抗-HBs、HBeAg、抗-HBe和抗-HBc均有相关(0.400<r<0.700,P<0.001),且呈中等相关。差异比较:HBsAg、抗-HBs和抗-HBc无统计学意义(P>0.05),HBeAg和抗-HBe有统计学意义(P=0.031和P=0.004)。结论 ECLIA和ELISA检测HBV-M五项指标有较好的符合性。ECLIA自动化程度高、检测范围宽。检测e系统较ELISA敏感。能定量检测,对评估HBV低水平感染及潜伏期感染、乙肝疫苗接种效果以及抗病毒治疗疗效判断具有重要的临床意义。
Objective To explore the clinical value,the advantages and difference between electrochemilu-minescenceimmunoassay(ECLIA) and enzyme linked immunosorbent assay(ELISA) to detect HBV-M in quantity.Methods 80 postive and 30 negative serum specimen were collected to detect the HBV-M(HBsAg,Anti-HBs,HBeAg,Anti-HBe and Anti-HBc) by the ECLIA and ELISA,to compare test results of the two methods.Resuts The coincidence rate between results from the ECLIA and ELISA was : 94.54% for HBsAg,95.45% for Anti-HBs,94.54% for HBeAg,80.00% for Anti-HBe and 95.45% for Anti-HBc.By the correlation analysis,the HBsAg,Anti-HBs,HBeAg,Anti-HBe,Anti-HBc was relevance(0.400r0.700,P0.001);by the difference comparison,there was no statistical significance Between HBsAg、Anti-HBs and Anti-HBc(P0.05);the statistical significance Between HBeAg and Anti-HBe(P=0.031 and P=0.004).Conclusions The compliance of detecting the five HBV-M indicators with ECLIA and ELISA is better.The ECLIA has high degree of autotion,wide detection range and the sensitivity of detecting the HBeAg and Anti-HBe is better than ELISA.In quantity,to assess HBV low level of infection and incubation infection,Hepatitis B vaccine,and antiviral therapy effect has the important clinical significance.
