胰腺实性假乳头状瘤的诊治体会

The Experience in Valid Diagnosis and Treatment of Solid Pseudopapillary Tumor of the Pancreas

目的探讨胰腺实性假乳头状瘤(SPT)的诊断和治疗。方法回顾性分析19例经手术病理证实的SPT的临床资料。结果 19例SPT中,男性2例,女性17例(其中10例为年轻女性);胰头部7例,胰颈部3例,胰体尾部9例。19例SPT均行手术切除治疗,手术方式为胰十二指肠切除术、肿瘤切除术、胰体尾切除术或胰体尾+脾脏+浸润灶切除术,无手术死亡病例。术后胰瘘2例,经保守治疗治愈。术后病理恶性者2例。随访5～52个月,未发现死亡、转移和复发病例。结论 SPT多见于年轻女性,CT是重要的诊断方法,治疗以手术切除为主,完整切除后预后良好。

Objective To investigate the valid diagnosis and treatment of solid pseudopapillary pancreatic tumors（SPT）.Methods The clinical data of 19 patients with SPT confirmed by pathology diagnosis were collected and analyzed retrospectively.Results Among the 19 cases,2 cases were males,17 cases were females（10 were young females）.There were 7 cases that the SPT located in the head of pancreas,3 cases in the neck of pancreas,and 9 cases in the body and tail of pancreas.All the cases underwent operation and without peri-operative death.The main treatment of SPT was surgical procedures including pancreatoduodenectomy,enucleation,distal pancreatectomy or distal pancreatectomy with splenectomy and metastatic lesion ectomy.Two cases of pancreatic fistula were observed,and were healed by use of medical treatment.The malignant SPT were found in 2 patients.All the patients were followed up for 5 months to 52 months.No evidence of death,relapses and metastases in these cases was found.Conclusion SPT is predominantly occurred in the young women,CT is an important diagnostic method of SPT,the surgery is the first choice of treatment.If the tumor is resected completely,the patients with SPT may get satisfactory prognosis.
