功能性甲状旁腺囊肿2例并文献复习

Clinical Analysis of Functioning Parathyroid Cyst:A Report of Two Cases and Review of Literature

目的探讨功能性甲状旁腺囊肿的临床特征,总结其诊治经验。方法回顾性分析2例功能性甲状旁腺囊肿患者的临床资料,并复习相关文献。结果 2例患者均为女性,年龄分别为55岁和60岁,病程分别为4个月和10个月,1例以颈部肿块为首发症状,两者均存在高血钙、高PTH和高血碱性磷酸酶血症。B超和CT检查2例均发现占位病变;1例行99mTc-MIBI甲状旁腺显像为阳性;术后病理2例均为腺瘤囊性变,其中1例疑似腺癌。结论单纯颈部包块为首发症状的患者不能排除功能性甲状旁腺囊肿;功能性甲状旁腺囊肿者不适于行穿刺抽液;对于甲亢症状典型且进展迅速的甲状旁腺囊肿患者在手术时应行快速冰冻切片,以尽早明确囊肿的良恶性。

Objective To further understand the characteristics of functioning parathyroid cyst,and to summarize the method of diagnosis and treatment.Methods The clinical data of 2 cases of functioning parathyroid cysts were analyzed retrospectively,and the literatures were reviewed.Results These 2 patients were all females.The age was 55 years old and 60 years old respectively.The duration was 4 months and 10 months respectively.One of them was admitted only for jugular lump.Hypercalcemia,high serum parathyroid hormone and high serum alkaline phosphatase were found.One was performed photon emission computed tomography imaging with 99mTc-MIBI,and it appeared positive.And the cysts were surgically removed and diagnosed by pathology.Conclusion A patient can＇t exclude functioning parathyroid cysts though his chief complaint is only jugular lump.Fine-needle aspiration isn＇t applicable in the diagnosis of functioning parathyroid cysts.And if a functioning parathyroid cysts＇s duration is short and the processes quickly,it should be suspected as malignant.
