Clinical Effectiveness of GTR Using Biomembrane in the Treatment of Periodontal Disease

摘要 观察国产可降解性胶原膜和非降解性膨体聚四氯乙烯膜、四氟乙烯六氟丙烯共聚膜引导牙周组织再生(GTR)的临床效果。方法:选择55例中~重度牙周炎患者63处病变区,随机分三组每组21处,分别置上述三种膜行GTR术。术前及术后6个月、12个月分别记录探诊深度(PD)、附着丧失(AL)、牙龈萎缩(RES)、牙龈指数(GI)、龈沟出血指数(SBI)、牙齿动度(TM)和摄X线牙片。结果:三组术后6个月、12个月的PD、AL、TM与术前比较有显著性差异;术后12个月,X线牙片分析,63处病变区骨密度增强为58.7%,三组的临床参数PD、AL、RES、GI、SBI组间均无显著性差异。结论:在GTR技术中,使用三种国产生物膜治疗牙周病损获得良好的临床效果,具有临床应用和推广价值。 Objective To evaluate the effectiveness of guided tissue regeneration (GTR) with one bioabsorbable and two non-resorbable membranes made in China in the treatment of periodontal diseases. Methods 63 sites of 55 patients with moderate to severe periodontal diseases were selected and randomly divided into 3 groups with 21 sites each. GTR was performed in three groups respectively using three membranes. Probing depth (PD), attachment loss (AL), gingival recession (RES), gingival index (GI), sulcus bleeding index (SBI), tooth mobility (TM) and X-ray check were recorded before and 6, 12 months after surgery. Results Clinical evaluations showed a marked attachment gain and significant decrease in PD and TM at 6th and 12th month after surgery. Radiographs' analysis revealed that increase of alveolar bone density was 58. 7% in 63 sites. No statistical difference was found among three types of barriers for PD, AL,RES,GI and SBI 12 months after surgery. Conclusions Periodontal diseases can be effectively treated by GTR with bioabsorbable and 2 non-resorbable membranes that are made in China.
