慢性肾衰血透患者血清和尿中IL-8的变化 被引量:5

摘要 目的:了解慢性肾衰(CRF)状态及血液透析(HD)环境下患者血清和尿中白细胞介素一8(IL-8)含量的变化规律和IL-8与残余肾功能(RRF)的关系.方法:采用双抗体夹心ELISA法检测25例CRF维持性血透患者和21例CRF非透析患者及26例健康体检者血清和尿中IL-8的含量.结果:CRF患者非HD组和HD组呶清和尿中IL-8的含量均显著高于正常对照组(P<O.01);HD组患者透析结束时血清和尿中IL-8的含量明显高于透析前(P<O.01)。血透结束后48h较血透结束时降低,但仍高于透析前,差别个显著(P>0.05);CRF患者非HD组与HD组血泊和尿中IL-8的含量均与Scr无明星相关性计(r=-0.34.P>O.05和r=-0.06.P>0.05);HD组中无尿患者(尿量<400mL/d)血清和尿中IL-8的含量明显高于有尿患者(尿量>400mL/d)(P<O.05).且患者血清和尿中IL-8的含量与尿量成显著负相关(r=一0.467,P<0.05和r=一0.548.P<O.05)结论CRF状态和HD环境下都存在严重的免疫炎症反应.且HD对IL-8的产生有明显的诱导作用,此可能为CRF维持性血透患者RRF进一步受损的主要原因之一。 AIM: To ()bscrvc the changes ()f 1I, 8 frorn serumand urlne ln hen1odialysis I,atients with chronic renal failure((:RF ) and study on the relationship between IL-8 to residualrcnal function (RRF ). METH0DS= Serum and urine 1l,-8levels were measured by Sandwich-EL1SA in 25 hemodialysispatients with CRF, 2l CRF patients without dialysis and 26healthy controls. RESULTS: The results showed that serumand urine II,-8 levels in hemodialysis patients with CRF andCRF patients without dialysis were significantly higher thanthose in healthy controls (P ty O. Ol ). In hemodialysis pa-tlents wlth CRF scrum and urine Il,tu8 leve1s after hemodialy-sis were significantly higher than those before hemodialysis(PwtO. OO1 ). Fourty eight hours after hemodialysis, the Iev-els decrease(l but were still higher than those beforehemodiaIysis (I'> O. O5 ). There was no significant c()rrelation betwccn serutn or urine II, 8 and serum crea (r =O. 3'1, PThO. O5 and r= - O. O6, P>O. O5). Serum andurine Il, 8 levels in hemodialysis patients with ur0n0nc()metryty 4OO mI,/day were significantly higher than those inhemodialysis I)atients with urononcometry > 4OO/day (P rO. O5 ). In hemodialysis patients with CRF, there was a nega-tive correlation between lhc serum and urine II,Al8 1evels andthe volurne of llrine (r= O. 467, PrO. O5 and r= - O. 548,PwtO. O5 ). C0NCLUSI0N: The hemodialysis patients withCRF and CRF patients without dlalysis all have seriolls im-mune lnflammation. Hemodialysis increases serum and urinelI, 8 levcls in hemodialysis patients with CRF, which may be()ne of the important causes leading to RRF darnage in patients with CRF on regu1ar hemodialysis.
