高压氧综合治疗小儿急性偏瘫临床观察

Hyperbaric oxygenation combining treatment,for children with acute hemiplegia

目的观察高压氧（ＨＢＯ）综合治疗小儿急性偏瘫的疗效。方法对照组给予常规治疗：抗感染，溶栓，扩血管，营养神经等。ＨＢＯ组在常规治疗的基础上加用高压氧治疗。婴幼儿用０．１８ＭＰａ单人纯氧舱，＞３岁儿童用０．２ＭＰａ多人高压氧舱，１００分钟／次，面罩吸纯氧３０分钟×２，中间休息１０分钟，每日１次，１０天为１疗程。结果ＨＢＯ组治愈率达８５％，显著高于对照组５９％（χ２＝４．８５，Ｐ＜０．０５），其治愈时间为（１６．５±３．３）天，显著低于对照组（２７．５±５．６）天（ｔ＝３．６９，Ｐ＜０．０１）。后遗症发生率为２％，显著低于对照组２０％（χ２＝４．３４，Ｐ＜０．０５）。结论高压氧综合治疗小儿急性偏瘫可提高治愈率，缩短病程，降低后遗症的发生，有推广价值。

Objective To investigate the curative effect of hyperbaric oxygenation(HBO) combining treatment on acute hemiplegic chlidren.Methods 1.Control group children(n=34)were only given the conventional medications including anti-infective drugs,thrombolytic agents,vasodilative drugs and brain cell activators etc.2.Experimental group children(n=39) were treated with HBO therapy besides above medications.The infants stayed in 0.18 MPa alone HBO chamber and children(>3 years) stayed in 0.2 MPa HBO chamber for 100 min.The pure oxygen with mask was inhaled for 60 min,interval 10 min,once a day.10 times were as a course of treatment.Results The cure rate of experimental group and control group was 85% and 59% respectively; the average time of treatment was 16.5±3.3 days and 27.5± 5.6 days; the sequalae were 2% and 20%.There was a significant difference in both groups(P<0.01).Conclusion It is showed that the HBO combining treatment can raise the cure rate,shorten the course of treatment and reduce the sequelae.It is worth spreading.
