高位结扎内翻剥脱联合经皮浅静脉连续环形缝扎术治疗下肢静脉曲张

High ligation and stripping of great saphenous veins combined with percutaneous continuous circumsuture of the varicose veins in the lower extremities

目的:观察在传统基础上改进的大隐静脉高位结扎内翻剥脱联合经皮浅静脉连续环形缝扎术治疗下肢静脉曲张的治疗效果。方法:随机选择下肢静脉曲张患者60例,采用大隐静脉高位结扎内翻剥脱联合经皮浅静脉连续环形缝扎术,观察和随访患者术后愈合和复发情况,包括对患肢溃疡复发的观察。结果:全部病例术后下肢静脉曲张均治愈,原曲张静脉全部消失,临床效果满意。56例68侧肢体得到随访,随访中发现小腿部静脉曲张复发5侧(7.3%),8侧(11.7%)和4侧(5.8%)在久立或行走后出现小腿酸胀感和足部肿胀,2侧伴有轻度疼痛。合并溃疡12例随访发现溃疡经久不愈2例,溃疡复发1例(8.3%)。结论:该方法操作简单,损伤小,瘢痕小,恢复快,疗效确切,不易复发。

Objective： To observe the therepeutic effect of high ligation and stripping of great saphenous veins combined with percutaneous continuous circumsuture for treating the varicous veins in the lower extremities.Methods： 60 atients with varicose veins in the lower extremities reveived high ligation and stripping of great saphenous veins combined with percutaneous continuous circumsuture,postoperative healing and recurrence,including ulcer recurrence of the involved legs,were observed and followed up.Results： The varicose veins in all patients were cured,all varicose veins disappeared,with clinical results satisfactory.This group of 68 limbs in 56 patients were followed up,5 cases（7.3%） of recurrent varicose veins in the calf were found,8 cases（11.7%） and 4 cases（5.8%） had a sense of soreness and foot swelling after a long time of standing or walking.2 cases had mild pain.12 patients with ulcers were followed up,2 cases had prolonged unhealed ulcer,1 case（8.3%） had ulcer recurrence.Conclusion：The procedure is a simple and less invasive therapy with small scar,rapid recovery,definite effect and less recurrence.
