弹性附着体义齿修复肯氏Ⅰ、Ⅱ类牙列缺损十年疗效评估

The ten-year clinical effect of restoring Kennedy Ⅰ and Ⅱ distal extension defect with resilience attachment

目的:评价弹性附着体义齿修复KennedyⅠ类和Ⅱ类末端游离端牙列缺损的长期临床效果。方法:采用Bredent VKS可变球和窝形附着体对KennedyⅠ、Ⅱ类39例牙列缺损患者进行修复,临床随访10-15年,观察其修复效果。结果:39例患者进行弹性附着体义齿修复设计后自觉义齿美观效果好,异物感小,固位稳定性好,咀嚼效能高。90%的患者远期临床修复效果好。其中1例患者在使用7年后基牙牙根折断,义齿松动,修复失败;少部分分别出现牙龈炎、牙髓炎等症状,经治疗后义齿恢复正常功能。结论:应用弹性附着体义齿修复KennedyⅠ、Ⅱ类牙列缺损有较好的远期疗效,但应按时随诊,及时发现和解决随访过程中出现的问题,以确保附着体义齿修复后的远期疗效。

Objective： TO investigate the long-term clinical effecf of Kennedy Ⅰ and Ⅱ distal extension partial denture with resiliencc attachments.Methods： 39 patient provided with 86 pieces of Bredent VKS attachments were followed up 10 years to 15 years,and were observed the restorative effect from subjective senes,oral and X-ray examination.Result： All patients with resilience attachment dentures attained esthetic,little sense of foreign body,good retention and stability,high mastication functioning,It had a good relative long-terrn clinical effect of 90% patients in more ten years.Following up 7 years,one patient had a bad effect because of the fracture of abutment root.After treating,The problems of gingivits,pulptis occurred in few patients.were resolved and the dentures wered returned to normal function.Conclusion： The manner of restoring KennedyⅠand Ⅱ distal dentures with resliience attachments have a good long-term clinical effect and is a practical method.It is extremely important to eusure the long term prosthetic effect with resilience attachment dentures,Which the problems were promptly discovered and solved and treated in the future consultatioin with a doctor.
