关节镜下逆行交锁髓内钉治疗股骨远端A型骨折

Analysis of the Treatment of Type A Dstal Femoral Fractures with Retrograde Interlocking Intramedullary Interlocking Nail Under Arthroscopy

目的 探讨关节镜下逆行交锁髓内钉治疗股骨远端A型骨折的临床效果及特点。方法23例股骨远端A型骨折患者采用关节镜下逆行交锁髓内钉内固定，闭合复位18例，小切口复位5例。术后早期予以CPM机锻炼。结果随访4～16个月，平均愈合时间4（2～8）个月。无畸形愈合和感染情况发生，膝关节功能恢复优良率达82．6％。结论关节镜下逆行交锁髓内钉治疗股骨远端A型骨折具有创伤小，对膝关节的干扰较少，同时提供坚强的内固定，膝关节功能恢复良好等优点。

Objective To investigate results of the treatment of type A distal femoral fractures with retrograde interlocking intramedullary interlocking nail under arthroscopy. Methods From 2007.7 to 2010.7 ,There were 23 cases of type A distal femoral fractures , treated with retrograde interlocking intramedullary interlocking nail under arthroscopy, 18 cases with closed reduction 5 cases reduce through a single percu -taneous, and accepted CPM exercise as early as possible after operation. Results All patients were followed up for 4 months to 16 months. All fracture healed with good functional recovery after 4 months average. No deformity, no infection, and the knee joint functions recovered well, and according to the Kumar scale, the excellent and good rate of of the knee joint function was 82.6%. Conclusions The treatment of type A distal femoral fractures with retrograde interlocking intramedullary interlocking nail under arthroscopy is less invasive to the soft tissues and knee, while providing firm union of fracture and it is conductive to the functional recovery of the knee joint.
