子宫动脉、螺旋动脉血流动力学变化在妊娠高血压综合征预测中的临床价值分析

Clinical value analysis on hemodynamics changes of uterine artery and spiral artery in pregnancy induced hypertension syndrome forecasted

目的探讨彩色多普勒测定妊娠高血压综合征(妊高征)患者子宫动脉、螺旋动脉血流动力学变化,以便找到一种早期预测妊高征的方法。方法 200例不同孕周的正常孕妇分7组测定子宫动脉、螺旋动脉阻力指数(RI)、搏动指数(PI)、收缩期与舒张期比值(S/D),分析其变化的规律,于孕≥32周,测定119例正常妊娠妇女与60例妊高征患者子宫动脉、螺旋动脉RI及PI值,并进行对比分析。结果中晚期妊娠螺旋动脉RI值基本稳定,33周以后子宫动脉RI值才基本稳定,妊高征组子宫动脉、螺旋动脉RI均明显高于正常妊娠组(P均<0.05),并以螺旋动脉更明显(P<0.01),60例妊高征患者中28例子宫动脉RI值增高占47%,43例螺旋动脉RI值增高占72%。结论子宫动脉、螺旋动脉血流动力学变化对妊高征有预测作用,且螺旋动脉优于子宫动脉。

Objective It is to approach the hemodynamics changes of uterine artery and spiral artery in pregnancy induced hypertension syndrome forecasted with color Doppler ultrasound to find a method for early forecast of pregnancy induced hypertension syndrome.Methods 200 normal gravida with different gestational weeks were divided seven groups to detect the resistent index（RI）,pulsatility index（PI）,the specific value of systole period to relaxing period（S/D） and analyze the regular pattern of the changes.The RI and PI of uterine artery and spiral artery of 119 cases of normal gravida and 60 patients with pregnancy induced hypertension syndrome were detected at ≥ 32 weeks of gestational weeks.The changes were compared and analyzed.Results The RI of spiral artery was mainly steadys at mid-later period pregnancy.The RI of uterine artery was mainly steadys after 33 weeks of gestational weeks.The RI of uterine artery and spiral artery in pregnancy induced hypertension syndrome group were both obviously higher than those in normal pregnancy group（both P0.05）,but the difference of spiral artery was more obvious（P0.01）.The RI of uterine artery of 28 cases（47%） in 60 patients with pregnancy induced hypertension syndrome was heightened.The RI of spiral artery of 43 cases（72%） in 60 patients with pregnancy induced hypertension syndrome was heightened.Conclusion The measurement of the hemodynamics changes of uterine artery and spiral artery has forecast action on pregnancy induced hypertension syndrome,and spiral artery is superior to uterine artery.
