摘要
目的了解沈阳市某高校职工不同诊断标准代谢综合征的患病状况,为修订标准和防治代谢综合征提供参考。方法以沈阳市某高校1 907名在职及退休职工为研究对象,进行人体测量、生理及血液指标检测。结果根据中华糖尿病学会标准(CDS标准)、美国国家胆固醇教育计划成人治疗指南Ⅲ修订标准(ATPⅢ修订标准)及国际糖尿病联盟标准(IDF标准),代谢综合征的粗患病率分别为22.2%3,5.5%及32.3%,标准化患病率为16.6%2,6.8%及23.8%,CDS标准下粗患病率、标准化患病率低于其他标准(P值均<0.01)。应用各标准,男性患病率高于女性,男、女患病率随年龄的增长而升高;在单项指标中,肥胖率较高;高三酰甘油、肥胖使代谢综合征的危险度明显升高。CDS标准与ATPⅢ修订标准及IDF标准的一致性中等,Kappa系数分别为0.64,0.62;调整年龄和性别后,BMI正常而腰围异常者患代谢综合征的危险度是2项均正常者的2.48倍(OR=2.489,5%CI为1.59~3.86,P<0.01)。结论该高校职工代谢综合征的患病率处于较高水平;3种标准对代谢综合征的诊断有差异,CDS标准诊断的患病率最低。
ObjectiveTo estimate and compare the prevalence of the metabolic syndrome(MS) by applying three different criteria.MethodsAnthropometric index,blood pressure and blood sample tests were measured among 1 907 staff in a university in northeast of China.MS criteria of Chinese Diabetes Society(CDS),National Cholesterol Education Program Adult Treatment Panel(modified ATP III) and International Diabetes Federation(IDF) were used in the analyses.ResultsBased on the CDS,modified ATP III and IDF criteria,crude prevalence of MS was 22.2%,35.5% and 32.3%,respectively.Age-standardized prevalence was 16.6%,26.8% and 23.8%,respectively.The crude and age-standardized prevalence diagnosed by the CDS were significantly lower than those diagnosed by the other two criteria(P〈0.01).Regardless of the criteria used,the prevalence in men was higher than that of in women,and increased with age in both genders.Obesity was the most common symptom in MS components.Agreements among CDS,modified ATP III and IDF were moderate with kappa values of 0.64 and 0.62.Risks in subjects with normal BMI and abnormal waist circumference were 2.48 times higher than those with the two components in normal ranges(OR=2.48,95%CI was 1.59-3.86,P〈0.01).ConclusionMS is highly prevalent in the university staff.The prevalence of MS diagnosed by the three criteria was different,with the CDS criterion the lowest.
