七氟烷或异丙酚复合瑞芬太尼麻醉对腹腔镜手术患者的影响 被引量:14

摘要 目的探讨七氟烷或异丙酚复合瑞芬太尼麻醉对腹腔镜手术患者血流动力学和血浆一氧化氮、内皮素-1的影响。方法将80例美国麻醉医师协会(ASA)分级为Ⅰ~Ⅱ级择期行腹腔镜手术的患者随机均分为七氟烷-瑞芬太尼组和异丙酚-瑞芬太尼组,两组患者分别使用七氟烷或异丙酚复合瑞芬太尼麻醉。术中和术后连续监测血压、心率和动脉血二氧化碳分压(PaCO2)的变化,并于麻醉前、气腹前、气腹后15 min和解除气腹后15 min抽取静脉血测定血浆一氧化氮和内皮素-1水平。结果气腹后15 min异丙酚-瑞芬太尼组平均动脉压和PaCO2均较麻醉前明显升高,七氟烷-瑞芬太尼组较麻醉前无明显差异,而较异丙酚-瑞芬太尼组明显降低;异丙酚-瑞芬太尼组气腹后15min内皮素-1浓度升高幅度明显高于七氟烷-瑞芬太尼组,而一氧化氮浓度升高幅度却明显低于七氟烷-瑞芬太尼组。结论腹腔镜CO2气腹可导致机体血流动力学平衡失调和血浆一氧化氮/内皮素-1升高;七氟烷复合瑞芬太尼麻醉较异丙酚复合瑞芬太尼可更好地维持血浆一氧化氮/内皮素-1的平衡,从而更好地维持血流动力学的稳定。
