慢性骨髓炎患儿父母主动要求截肢原因的质性研究

A Qualitative Study on the Reasons of Voluntary Requesting Amputation for Parents of Children with Chronic Osteonuelius

目的深入探讨胫腓骨开放性骨折术后感染骨髓炎患儿家属主动要求截肢的原因。方法运用质性研究中的参与式观察法、资料分析法及深度访谈法对一主动要求截肢家庭做出该决定的原因进行研究,运用Claizzi现象学资料7步分析法进行资料分析和整理。结果患儿家属主动要求截肢的原因:疾病本身的特点、经济困难、来自各方面不确定的声音、从孩子的发展出发、首诊医生留下的烙印及家属自主意识增强。结论疾病本身的特点是研究对象做出截肢决定的医学原因;经济困难及从孩子的发展出发为社会原因;来自各方面不确定的声音及首诊医生留下的烙印为医护人员与患者共同的原因;家属自主意识增强是患者方面的原因。

Objective To discuss the reasons of voluntary requesting amputation for parents of children with Chronic Osteonuelius.Methods Participant observation,data analysis and in-depth interview were applied in this study.Data were analyzed by Claizzi＇s seven-step analysis method.Results The reasons of voluntary requesting amputation for parents of children included the characteristic of the disease itself,economic difficulties,uncertain sound from others,considering the child＇s development,impression of the initial doctor and enhanced self-awareness.Conclusion The disease is the medical cause for the decision of amputation.Economic difficulties and consideration for children＇s development are the social cause.Uncertain sounds from others and impression of the initial doctors are the common causes for the medical staff and patients.Enhanced self-awareness is the cause of patients.
