摘要
目的:评价乏血供结肠癌肝转移瘤行肝动脉介入治疗的临床疗效和影响因素。方法:回顾分析25例结肠癌肝转移瘤患者数字造影血管造影(DSA)的表现、介入治疗方案的选择、疗效评价和预后因素。结果:25例均为多发转移瘤,DSA造影多为瘤体中心乏血供的环形染色。共行74次介入治疗,其中单纯灌注化疗21次,化疗加碘油栓塞53次,0.5,1,2,3年生存率分别为88%,72%,32%,8%。结论:肝动脉介入治疗能有效提高乏血供结肠癌肝转移患者的生存期,应为首选治疗办法。
Objective：To evaluate the clinical effect and influencing factor of interventional therapy by hepatic artery in the treatment of lackvascular hepatic metastasis from colonic carcinoma.Methods：To analyse digital subtraction angiography（DSA） images,interventional therapeutic regime,the therapeutic effectiveness and prognosis factor for 25 patients with hepatic metastasis from colonic carcinoma.Results：All of them were multiple metastasis and lackvascular in the central area of tumour.A total of 25 patients was treated 74 times by interventional therapy.Of the 74 times,only infusion chemotherapy was performed at 21 times,and infusion chemotherapy and lipiodol embolization at 53 times.The 0.5,1,2,3 and 5 years survival rate of standard group was 88%,72%,32%,8%.Conclusion：Interventional therapy by hepatic artery can effectively improve survival period of lackvascular hepatic metastasis from colonic carcinoma and it is the first method.
