2007～2009年云南潞西市疟疾防治效果评价

Evaluation of results of malaria control in Luxi City,Yunnan Province in 2007～2009

目的了解潞西市2007～2009年全球基金疟疾项目地区疟疾流行与防治状况，评价项目的实施效果。方法2007年和2009年采取整群分层随机抽样法对抽到村寨的居民采血镜检，入户调查每户家庭拥有药浸蚊帐的数量，对调查日期前两周内的所有疟疾病人按个案调查表的内容要求进行个案调查，查阅调查村寨所有村医门诊登记和处方，获得该诊所疟疾病人的有关信息；然后在乡（镇）防保组核查该村诊所上报的疟疾病人（包括疑似病例）的有关信息。结果居民带虫率从2007年的1．36％下降到2009年的0；2006年、2009年拥有药浸蚊帐（包括长效蚊帐）的家庭比例分别是27．79％、74．10％；项目地区疟疾病例在一周内从村级报告到乡镇的比例分别是12％、59．64％；调查的前两周内的疟疾病人在发热24h内得到抗疟药规范治疗的比例分别是8．33％、52．00％，均有显著性差异。结论第五轮全球基金疟疾项目开展的各项防制措施对潞西市疟疾的流行有显著的控制作用，今后仍应继续完成好项目的各项指标使潞西市的疟疾发病进一步下降。

Aim To understand malaria situation for evaluation of effect after implementing the fifth round malaria control program of global fund in Luxi City, Yunnan Province. Methods Stratified random cluster sampling method was used to collect the information in each family. The blood spots of each resident were taken, and questionnaire survey of the number of impregnated bednets in each family and the information of malaria cases in two weeks was performed. The records and prescription of for treatmen of malaria cases in village clinics were examined. Meanwhile,the records of malaria cases including suspected malaria cases reported by village doctors were checked at township level. Results The malaria incidence decreased from 1.36% in 2006 to 0 in 2009. The rates of possessing impregnated bednets （including LLINS） in a family were 27.79% ,74.10% in 2006 and 2009 respectively. The malaria cases were reported at village and township levels were 12.00% and 59.64% respectively. The standsard treatment rates of malaria cases in two weeks after detected were 8.33% and 52.00% , respectively, showing statistically significant difference （P〈0.01 ）. Conclusion The implementation of malaria program is effectgive for controlling the prevalence of malaria in Luxi City and a lot of work has to be done to attain the indicators for further reduction of malaria incidence.
