COPD并发呼吸衰竭的常见诱因及其死亡危险因素分析

COPD with respiratory failure:common causes and risk factors of death

目的探讨慢性阻塞性肺疾病(COPD)常见诱因及引起死亡的危险因素。方法对78例COPD患者的临床资料进行回顾性分析。结果 78例COPD住院患者发生呼吸衰竭45例。呼吸衰竭发生和每年发作次数、全身营养状况、COPD的分级、有无吸入糖皮质激素、酸碱平衡紊乱、高血糖、有无真菌感染等因素有关。呼吸衰竭发生和与年龄、病史时间无关。呼吸衰竭死亡组中,合并心力衰竭、肺性脑病、酸碱平衡紊乱率高达100%。结论 COPD常见诱因复杂,并发呼吸衰竭死亡危险明显增加。

Objective To investigate common causes and risk factors causing death of the chronic obstructive pulmonary disease（COPD）.Methods The data of 78 cases of COPD patients from June 2007 to June 2010 were retrospectively analyzed.Results Out of 78 hospitalized COPD patients,45 cases had respiratory failure.The morbidity of respiratory failure was associated with the annual times of episodes,systemic nutritional status,COPD classification,inhaled corticosteroids application,acid-base balance disorders,high blood sugar,fungal infections and other factors.Respiratory failure had no relationship with age and the course of the disease（P0.05）.In the group who died of respiratory failure,the rate of combined complications including heart failure,pulmonary encephalopathy and acid-base disturbance was as high as 100%.Conclusion The causes of respiratory failure in COPD were complicated and risk of death respiratory failure was significantly increased.
