血清结核抗体检测及结核菌素试验对肺结核病诊断的价值

Detection of serum anti PPD lgG and tuberculin test in the diagnosis of Pulmonary Tuberculosis

目的 评价血清抗 PPD Ig G检测及 PPD皮试在肺结核病诊断中的价值。方法 对162例肺结核病患者 ,40例非结核病患者的 PPD皮试结果及血清抗 PPD Ig G检测结果进行相关回归分析及 x2 检验。结果 PPD皮试敏感度为 72 8% ,特异度为 60 % ;血清抗 PPD Ig G检测敏感度为 79% ,特异度为 5 5 % ,两者联合检测敏感度 89 5 % ;双阳性的特异度 85 % ;双阴性排除肺结核的特异度 97%。有无空洞对两者阳性率的影响均有显著性差异。相关回归分析显示两者无显著相关性。结论 PPD皮试联合血清抗 PPD Ig

Objective To ascertain the value of serum annti PPD IgG detection and tuberculin test in the diagnosis of pulmonary tuberculosis.Methods The data of serum anti PPD IgG detection and PPD skin test results,which were obtained from 162 cases with pulmonary tuberculosis,40 cases with nontuberculosis,were assessed by chi square test and correlation regression analysis.Results For PPD skin test,the sensitivity and specificity were 72 8% and 60%,and for anti PPD IgG detection,they were 79% and 55% respectively,while for PPD skin test combing serum anti PPD IgG detection,the sensitivity was 89 5%.The simultaneous positive specificity were 85%,but the negative ones were 97%.Having cavity or not affected the positivities greatly.The correlation regression analysis.of diameter of PPD skin test and positivity of serum anti PPD IgG detection showed there is no significant correlation between the size and positivity.Conclusions The combination of serum anti PPD IgG detection and PPD skin test.could augment the diagnostic sensitivity and specificity obviously.
