中药、化疗配合沙利度胺治疗多发性骨髓瘤回顾性疗效分析

Retrospective effect analysis of integration of traditional Chinese medicine and western medicine in treatment of multiple myeloma

目的通过对中药、化疗配合沙利度胺治疗多发性骨髓瘤（multiple myeloma，MM）回顾性的疗效分析，以期发现对高龄多发性骨髓瘤患者缓解率高，生存期长，生活质量好的治疗方案。方法临床以是否使用沙利度胺为条件，将患者分为两组，治疗组14例采用“中药＋常规化疗＋沙利度胺”治疗，对照组13例采用“中药＋常规化疗”治疗。结果治疗组总有效率为71．4％，高于对照组的38．5％，但差异无统计学意义（P=0．128）；两组的中位生存期分别为21．8个月与12个月，差异有统计学意义护=0．001）；两组的3年和5年生存率分别为28．6％和15．4％，7．1％和0．0％，但差异无统计学意义∽值分别为0．317、1．000，P均〉0．05）。结论治疗组患者可获得较高的完全缓解（cR）或部分缓解（vGPR），对于高龄或无移植条件的年轻MM患者是较好的治疗选择。

Objective To find out a treatment with high remission rate, long living period, and a good quality of life for elderly patients with multiple myeloma. Methods All patients were recruited into a treatment group （treated with traditional Chinese medicine, routine chemotherapy, and thalidomide） and a comparison group （treated with traditional Chinese medicine and routine chemotherapy）. Results The total effective rate of the treatment group was 71.4%, higher than the comparison group （38.5%）, but showing no statistical difference （P= 0.128 〉 0.05）. Median survive time of two groups were 21.8 months and 12 months respectively, （P=0.001 〈0.01）. The survival rate of 3 years and 5 years were 28.6%, 15.4% and 7.1%, 0 in the two groups respectively, without significant difference （0.317, 1.000, both P〉0.05）. Conclusion The treatment group showed higher results in both complete remission rate （CR） and very good partial remission rate （VGPR） than the comparison group, demonstrating a better results in improving the patient＇s quality of life. The treatment group also had a higher value of the median survive time and the median progression-flee surial time than the comparison group. The combined therapy of traditional Chinese medicine, routine chemotherapy and thalidomide is an ideal choice for both aged patients or young MM patients who had no transplant conditions.
