小儿过敏性紫癜血液灌流治疗72例分析

Efficacy Analysis of Hemoperfusion in 72 Children with Henoch-Schonlein Purpura

目的:探讨血液灌流治疗小儿过敏性紫癜的临床疗效。方法:对2009年1月～2010年10月在我院住院行血液灌流治疗的72例过敏性紫癜患儿入院前的治疗、血液灌流前后临床资料及随访情况等进行回顾性分析。结果:(1)72例中62例(86.1%)有院外药物治疗经历,59例无明显好转,占95.2%。在用药史明确的32例患儿中,24例(75.0%)使用过激素。住院期间64例(88.9%)使用了短程静脉糖皮质激素治疗。(2)血液灌流治疗的入选标准:皮疹症状严重,反复发作,消化道症状明显,血管神经性水肿明显。(3)血液灌流对过敏性紫癜急性期炎症反应所致的临床症状缓解效果明显,无预防皮疹复发的作用,对肾脏损害无明显的保护作用。(4)随访中,大多数患儿家长对血液灌流缓解过敏性紫癜急性期炎症反应所致的临床症状的疗效表示满意。结论:血液灌流是一种能迅速缓解过敏性紫癜急性期血管炎症反应所致临床症状的安全治疗方法,且价格适中,患儿家属易接受。

Objective： To explore the clinical curative effect of hemoperfusion with Henoch-Schonlein purpura in children.Methods：Clinical data of total 72 patients with Henoch-Schonlein purpura treated by hemoperfusion during January 2009 to October 2010 in our hospital were retrospectively analyzed.Results：（1） Sixty-two cases（86.1%） received extramural hospital drug treatment,in which 59 cases（95.2%） had no significant improvement.Sixty-four cases used a short-range vein glucocorticoid therapy in our hospital.（2） Hemoperfusion treatment criteria： serious or（and） repeated rash,serious gastrointestinal symptoms,serious angioneuroedema.（3） Hemoperfusion had obvious effect on easing clinical symptoms caused by acute inflammation in Henoch-Schonlein purpura,no preventive function on rash recurrence or kidney damage.（4） Most patients＇ parents were satisfied with the curative effect of hemoperfusion.Conclusions：Hemoperfusion is a safe therapy to ease clinical symptoms caused by acute inflammation in Henoch-Schonlein purpura,which is moderate priced making it affordable for families.
