腹泻病高发地区人群医疗保险模式与就诊意愿及行为分析

Study on medical insurance model and medical seeking willingness and behavior in population of high diarrheal areas

目的了解腹泻病调查地区人群医疗保险模式以及对就诊意愿、行为的影响。方法在腹泻病入户调查地区对部分健康人进行基本情况、有无医疗保险、模式以及就诊意愿和行为的调查。结果医疗保险模式城镇和农村人群分别以不享受任何医疗保险(44.72%)和农村合作医疗为多(53.21%);农民和公务人员分别以农村合作医疗为主(56.10%)和以公费医疗(59.50%)为主,其余职业人群均以不享受任何医疗保险的比例为高;调查人群认为患病后自己买药治疗、不必治疗和到医院就诊的分别占76.73%、8.45%和38.82%;患病后认为自己买药治疗和不必治疗的以不享受医疗保险人群为多,愿到医院就诊则以公费医疗保险者为高;腹泻病例就诊率以不享受任何医疗保险人群(36.41%)和农村合作医疗保险(24.18%)为高。结论农民、社区居民等不享受医疗保险弱势群体对于腹泻病等常见病的就诊意识不强,但就诊需求高。须提高农民、社区居民的医疗保险参保意识,将常见病纳入保障范围,加强基层医疗服务体系的建设,提高就诊意识。

Aim To survey medical insurance model and the impact on medical seeking willingness and behavior in population for diarrheal survey.Methods A household survey was conducted in study area including the contents of basic information,medical insurance status,medical seeking willingness and behavior.Results There 44.72%of the urban residents and 53.21%of the rural inhabitants surveyed did not enjoy any medical insurance and rural cooperative medical care.The coverage of rural cooperative medical care and public health services were 56.10%and 59.50%.The diarrheal cases surveyed relied on self medication,giving up medical treatment and treatment in hospitals accounmted for 76.73%,8.45%and 38.82%.Most of the diarrheal cases enjoying public medical service went to hospitals for treatment,while only 36.41%not enjoying public medical service and 24.1%enjoying rural cooperative medical service went to hospitals for treatment.Conclusion Farmers,community residents and other vulnerable groups not enjoying medical insurance are not aware of common diseases such as diarrhea strongly,but the demand for attendance is high.
