PICC置于肘下近端背侧贵要静脉的肢体位改进

The Reforming Body Position in Placing Catheter at the Proximal Dorsal Basilic Vein below Elbow in Patients with PICC

目的探讨PICC置于肘下近端背侧贵要静脉改进肢体位的可行性。方法置管部位选为肘下近端背侧贵要静脉,在改进肢体位及体位下,执行穿刺、送管、维护和拔管。结果改进组37例与同期常规组163例作比较,在穿刺、送管、维护、拔管的容易程度上无明显区别;在导管头端位置方面,穿刺成功率高,送管顺利,导管头端均能到达上腔静脉下端的理想位置。结论肘下近端背侧贵要静脉采用改进组方法实施PICC置管,导管头端到达理想的上腔静脉下端,无临床感染,故认为改进组方法是可行的。

Objective To explore the feasible approach of placing catheter at the proximal dorsal basilic vein below elbow by reforming body position in patients with PICC. Methods To divide the patients into observation group and control group according to different body positions when placing catheter at the proximal dorsal basilic vein below elbow, and evaluate the feasibility of reforming body position. Results Thirty seven cases were included in this study and the cases of mechanical phlebitis in the observation group was less than that in the control group. The difference had statistical significance. Conclusion In the modified approach, puncturing, placing, preserving and drawing the catheter could be finished successfully, and the head of catheter could get the ideal inferior extremity of superior vena cava without infection. The approach of reforming body position could decrease the rate of mechanical phlebitis and is better.
