重症监护病房住院患儿医院感染的多因素分析

Multiple factors analysis of nosocomial infection of hospitalized children in intensive care unit

目的通过对重症监护病房住院患儿医院感染发生情况进行研究,探讨其危险因素,为预防和控制医院感染提供依据。方法以2009年6-11月河北省儿童医院重症监护病房住院的872例患儿为研究对象,确定医院感染病例,并设立对照组;将资料进行多因素logistic回归分析。结果 872例患儿中,发生医院感染者101例,感染率为11.58%;医院感染多因素分析发现,基础疾病>3个、抗菌药物应用种类>2种、早产儿、机械通气等是明显的医院感染危险因素。结论合理使用抗菌药物、减少侵入性操作,将有利于预防和控制儿科医院感染;充分了解医院感染的高危因素,对易感者实施重点保护,有助于减少医院感染的发生。

OBJECTIVE To find out the nosocomial infection risk factors through analyzing the situation of nosocomial infection of the children in the ICU of the specialized children′s hospital and supply theoretic basis for prevention and control of nosocomial infection.METHODS A total of 872 patients in ICU of the specialized children′s hospital during Jun 2009 Nov 2009 were selected as subject to definite nosocomial infection cases.A case-control study was conducted.The case control group was set up.the influence factors of nosocomial infection in the ICU of the specialized children′s hospital were analyzed by multivariate logistic regression.RESULTS Of 872 cases of inpatients,101 cases occurred to the nosocomial infection with the infection rate of 11.58%;the results of multivariate logistic regression indicated that more than 3 foundation disease,more than 2 kinds of antibiotics categories,premature birth and mechanical ventilation were the obvious high-risk factors.CONCLUSION Reasonably age of antibiotics,reducing the invasive operation are advantageous to the prevention and control of nosocomial infection.Understanding the high-risk factors of nosocomial infection and protecting the susceptible patient target-orientedly are helpful in reducing nosocomial infection.
