摘要
虽然结直肠癌病人的5年存活率有所提高,但接受根治术后仍然有33%的直肠癌病人将面临局部复发的威胁,并且这部分病人的治疗给肿瘤科医生提出了很大的挑战。联合脏器切除的手术治疗仍然是提高5年存活率的主要治疗方法,R0切除是判断病人预后最有力的因素。腹-骶联合脏器切除术由于并发症发生率和病死率过高,无明显生存期的延长,现已被很多外科医师摒弃。术前、中的放化疗联合手术的多学科综合治疗提高了局部复发直肠癌R0切除。
Although the five-year survival rate of patients with colorectal cancer has improved, there are still 33% of patients performed radical resection of rectal cancer who will face the threat of local recurrence. The treatment of the patients presented to oncology doctors great challenges. The surgical treatment of combined organ resection is still the main treatment method to improve five-year survival rate. R0 resection is the most powerful prognostic factor. Abdomen-sacral combined organ resection with exorbitant complication incidence and mortality rate and no significant prolongation of survival has been abandoned by many surgeons. Preoperative and intraoperative chemoradiotherapy combined with surgery in the muhidisciplinaty treatment could improve RO resection of locally recurrent rectal cancer.
