奥氮平配合格拉斯琼预防恶性肿瘤化放疗所致恶心、呕吐临床疗效观察

Clinical Observation of Olanzapine Combined with Granisetrom and Hexadecadrol Prevent Nausea Vomit Induced by Chemoradiotherapy

目的:观察奥氮平配合格拉斯琼及地塞米松联合预防化放疗所致恶心、呕吐的临床效果及毒副反应。方法:采用随机交叉、自身对照法,将患者分成两组:A组第一周期用格拉斯琼及地塞米松;B组第一周期用奥氮平与格拉斯琼及地塞米松;第二周期A、B组交叉使用。观察46例恶性肿瘤患者自身先后二次化疗对奥氮平、格拉斯琼、地塞米松预防恶心、呕吐的程度及疗效。结果:治疗组(联合奥氮平):显效38例(82.6%),有效4例(8.7%),微效3例(6.5%),无效1例(2.2%),总缓解率为97.8%;对照组(未用奥氮平):显效15例(32.6%),有效18例(39.1%),微效6例(13.0%),无效7例(15.2%),总缓解率为(84.8%)。两组疗效差异有统计学意义(x^2=4.93,P<0.05)。化疗期间其他不良反应主要为便秘(分别为32.6%和34.8%),其次为头晕(分别为23.9%和21.7%)、疲倦(均为19.6%)、烦躁(分别为4.3%和2.2%)等。两组的不良反应发生率差异无统计学意义(P>0.05)。结论:奥氮平与格拉斯琼、地塞米松联合对恶性肿瘤化放疗所致恶心、呕吐较格拉斯琼、地塞米松具有更明显的疗效,且无明显的毒副作用。

Objective：To observe the clinical effect of Olanzapine combined with Granisetrom and Hexadeeadrol in prophylaxis of nause and vomiting by chemoradiotherapy.Methods：Using random cross,Self-reflection methods.Random divided patients into two groups.A group of the first cycle use granisetrom and hexadecadroI,Top of group B with olanzapine combined with granisetrom and hexadecadroLThe second cycle of A and B groups cross use.To observe the clinical effect of 46 cases of malignant tumor patient itself has two chemotherapies for olanzapine,granisetrom and hexadecadrol prevention nausea and vomiting.Results：Treatment group（united olanzapine）：38 patients got significant effect（82.6%）,4 effective（8.7%）,3 minor effect（6.5%）,l invalid（2.2%）,the total response rate was 97.8%;The control group（olanzapine is not used）： 15 patients got significant effect（32.6%）, 18 effective（39.1%）,6 minor effect（1 3.0%）,7 invalid（l 5.2%）,the total response rate was 84.8%.There was statistically significant difference between two groups（g2=4.93,P〈O.OS）.During chemotherapy other adverse reactions were constipation（32.6% and 34.8%,respectively）,followed by dizziness（23.9% and 21.7%,respectively）,fatigue（19.6% both）,irritability（4.3% and 2.2%,respectively）,etc.Adverse reaction rates in both groups have no significant difference（P〉0.05）.Conelusion： Clinical observation therapeutic of olanzapine combined with granisetrom and hexadecadrol in malignant tumor chemotherapy induced nausea,vomiting have more obvious curative effect compared to granisetrom and hexadecadrol,and no significant side effects.
