术期未输血及输血对口腔癌患者T细胞亚群的影响

THE EFFECT OF BLOOD TRANSFUSION AFTER OPERATION ON T-CELL SUBGENUS OF PATIENTS WITH ORAL CAVITY CANCER

目的 探讨术期未输血对Ｔ细胞亚群的影响；方法 应用Ｔ细胞单克隆抗体ＯＫＴ系列检测了４２ 例口腔癌患者及２０ 例口腔颌面部良性肿瘤患者。其中检测了２２ 例术期未输血和２０ 例术期输血的口腔癌患者的Ｔ细胞亚群。结果 癌瘤患者的ＣＤ３ 、ＣＤ４／ＣＤ８ 值比健康人、口腔良性肿瘤患者明显降低、而ＣＤ８ 值却明显升高；术后ＣＤ８ 值降低，ＣＤ４／ＣＤ８ 值升高。对４２ 例口腔癌患者术后的Ｔ细胞亚群进行了动态观测，术期未输血患者及术期输血患者在术后第３ 、７ 天，ＣＤ３ 、ＣＤ４ 值下降，ＣＤ８ 值降低；术后第１４ 天，未输血组患者ＣＤ３ 、ＣＤ４ 、及ＣＤ８ 值，ＣＤ４／ＣＤ８ 值恢复至术前水平；输血组患者ＣＤ３ 、ＣＤ４ 、ＣＤ８及ＣＤ４／ＣＤ８ 值未恢复至术前水平；术后第２１ 天，未输血组ＣＤ４／ＣＤ８ 值恢复正常而输血组未恢复正常。结论 输血可影响患者免疫功能的恢复进程。

Objective: To study the effect of blood transfusion after operation on T-cell subgenus of patients with oral cavity cancer. Methods: We detected 42 cases of oral cavity cancer, and 20 cases of benign tumor of oral cavity with OKT series of T-cell monoclonal antibody. The T-cell subgenus of 22 cases of oral cavity cancer that did not receive any inter-operation blood transfusion. and that of 20 cases of oral cavity cancer that received inter-operation blood transfusion was detected. Results: It was found that the CD3, CD4/CD8 in patients with oral cavity cancer is obviously lower than that of patients with begnign oral cavity tumor; and the CD8 is obviously higher. We have detedted the T-cell subgenus of 42 cases of oral cavity cancer after operation , and found that CD3,CD4 and CD8 were lower on 3rd, 7th day after operation than before, and CD3, CD4, CD8 and CD4/CD8 turned to the pre-operation level on the 14th day after operation in the patients that did not received any inter-operation blood transfusion; but CD3, CD4, CD8 and CD4/CD8 did not turn to the normal level in theose patients that reveived inter-operation. Conclusion: It demonstrated that inter-operation blood transfusion can delay the recovery of immunity system.
