我院11341张处方中不合理处方点评

目的:了解我院不合理用药处方的现状,提高药师参与临床合理用药的能力。方法:随机抽取我院2010年1～5月每月3天门诊及住院处方共计11341张,从中挑选出不合理处方按照《医院处方点评管理规范(试行)》评价标准进行归类分析。结果:不合理处方813张,不合格率为7.17%。不合理处方发生频率较高的是处方书写不规范及药物临床使用的适宜性(用药适应证、药物选择、给药途径、用法用量、药物相互作用、配伍禁忌等)。结论:我院存在一定程度的不合理用药现象,主要原因是由临床医生缺乏足够的药物知识所引起,作为药师应具备药学综合性知识,协助医生做好药物临床的合理使用工作。

Objective： to understood that our hospital＇s unueasonable medication prescription＇s present Situation, enhances the pharmacist to participate in the clinical reasonable medication ability. Methods： Random sample of our hospital in January 2010 to May of 11341 prescriptions, seleered from unreasonable prescriptions in accordance with the ＂Hospital Practices Prescription Comment＂ classified analysis of the evaluation criteria.Results： The unqualified prescription has 813, the fraction defective is 7.17%. The main existence＇s question is the prescription writing not standard and the medicine clinical use suitability （adaptive medication, drug selection, route of administration, dosage, drug interactions, incompatibility, etc. ）. Conclusion： Our hospital existence certain extent＂ s unreasonable medication phenomenon, the primary cause is because clinician lacks the ehough medicine knowledge,phamacists should have a comprehensive knowledge of medicine to help physicians make rational use of drugs of clinical work.
