结核病住院患者发生医院感染的临床分析

Clinical analysis on nosocomial infection of patients being in hospital with Tuberculosis

目的了解结核病住院患者医院感染特点,分析其原因,为预防与控制结核病患者医院感染提供科学的理论依据。方法 采用前瞻性调查结合回顾性调查方法,对我院2010年结核病住院患者发生医院感染病例的临床资料进行分析。结果 2904例患者中有201例发生医院感染,感染率6.92%,发生感染208例次,例次感染率为7.16%;感染部位以下呼吸道感染最高为72.35%,泌尿道感染(8.79%)次之;引起医院感染的病原菌以真菌为主占46.91%,G-菌次之占37.04%;真菌感染以白色念珠菌为主。感染与结核患者住院时间长、基础疾病、抗生素及抗结核药物的使用、各种侵袭性操作及患者的免疫功能有关。结论结核病患者易发生医院感染,必须加强医院感染预防工作,减少感染的发生。

Objective To study the clinical feature of nosocomial infection on Tuberculosis patients hospitalized and analyse the correlative reasons, to offer the scientific theory basis for preventing and controlling the nosocomial infection of Tuberculosis patients hospitalized. Methods A survey of prospective combination retrospective was undertaken in data of tuberculosis patients hospitalized cases during 2010. Results 201 nosocomial infections in 2 904 patients were analysed.The infection rate of nosocomial infection was 6.92%,208 nosocomial infections cases in 2 904 patients, infection rate of nosocomial infection cases was 7.16%; the highest infection site was lower respiratory tract（72.35%）,the urological infection accounted for 8.79%;among pathogens isolated from nosocomial infection cases, fungi accounted for 46.91%, Gram-negatives accounted for 37.04%. It was the highest infection of white beads bacteria among fungal infection, infection rate was related tuberculosis patients hospitalized for a long time,the underlying disease, antibiotics and resist the use of TB, the method of operation and the patient＇s immune functions. Conclusion It is easy for Tuberculosis patients hospitalized to get nosocomial infection, we must strengthen the work of prevention of infection,reduce nosocamial infection.
