超声对糖尿病患者颈动脉改变及左心室功能改变关系的研究

Study on the relationship between changes in carotid artery and left ventricular function in diabetes patients in ultrasound

目的:探讨糖尿病患者颈动脉改变与左心室功能改变的关系。方法:对215例糖尿病患者和178例健康人行颈动脉及左心室超声检查分析。结果:糖尿病组215例患者中颈动脉内膜与中层(intima-media thickens,IMT)增厚者为203例,占94.4%,粥样硬化斑块形成146例,共312处,IMT值(P<0.05)及粥样硬化斑块数目较对照组明显增高(P<0.01),颈总动脉窦部是动脉粥样硬化斑块的好发部位,共186处。糖尿病组患者左室舒张及收缩功能均较对照组明显降低(P<0.05),糖尿病组病程,大于5年患者较病程小于5年患者粥样硬化斑块数目增多(P<0.05),左室收缩功能降低更明显(P<0.05)。结论:糖尿病患者颈动脉结构变化与左心室功能改变存在相关性,利用超声行颈动脉及左心室检查,对糖尿病患者合并心脏功能改变的早期诊断具有指导意义。

Objective：To evaluate the relationship between changes in carotid artery and left ventricular function in diabetes patients.Methods The ultrasound results for carotid artery and left ventricular of 215 cases patients in diabetes and 178 cases normal persons were analyzed.Results The carotid arteries intima-media thickness in 203 cases were significantly thickened（94.4%,203/215）,there were 312 carotid arteries hard plaque in 146 cases,the arteries intima-media thickness（P0.05） and the number of carotid arteries hard plaques in diabetes group were significantly higher than that in control groups（P0.01）;There are 186 plaques in the bifurcating part of carotid,where is likely to be the good site that these hard plaques happen.The left ventricular diastolic and systolic function in diabetes group are significantly lower than that in control groups（P0.05）,the number of carotid arteries hard plaques in course of diabetes disease 5 years group were significantly higher than that in course of disease 5 years group（P0.05）, and the left ventricular systolic function in course of diabetes disease 5 years group decreased significantly too（P0.05）.Conclusions The carotid change was correlated with left ventricular function in diabetes patients,it is instructive using ultrasonography of carotid artery and left ventricular for the patients in diabetes with early cardiac function changes diagnosis.
