急性扁桃体炎发热病例的证候类型及特点

Syndrome types and characteristics of fever cases of acute tonsillitis

目的进行急性扁桃体炎发热病例的证候类型分类以及症状与体征特点描述。方法应用统一的证候调查表完成233例入选病例的问卷调查,按照病邪之不同将其分为温热类或湿热类,温热类病例采用六经辨证,湿热类采用卫气营血辨证。结果温热类病例(207例)可分为太阳病(21例,占10.1%)、太少合病(34例,占16.4%)、少阳病(2例,占1.0%)、太明合病(46例,占22.2%)、三阳合病(76例,占36.7%)、阳少合病(9例,占4.4%)、阳明病(19例,占9.2%)7种证候类型,湿热类(26例)可分为卫分证(5例)、气分证(2例)、卫气合病(19例)3种证候类型。结论急性扁桃体炎发热病例可归纳为10种有规律可循的证候类型,以三阳合病、太明合病、太少合病较为常见。

Objective To describe the syndrome types and characteristics of symptoms and signs in fever cases of acute tonsillitis.Methods The fever cases of acute tonsillitis（n=233） were investigated by using unified syndrome questionnaire,and then they were divided into warm-heat type and dampness-heat type according to the pathogenic factors.The cases of warm-heat type were given six-meridian syndrome differentiation,and those of dampness-heat type were given defense-qi-nutrient-blood syndrome differentiation.Results The cases of warm-heat type（n=207） could be divided into 7 syndrome types including Taiyang diseases（n=21,10.1%）,disease involving both Taiyang and Shaoyang（n=34,16.4%）,Shaoyang diseases（n=2,1.0%）,disease involving both Taiyang and Yangming（n=46,22.2%）,disease involving Taiyang,Shaoyang and Yangming（n=76,36.7%）,disease involving both Yangming and Shaoyang（n=9,4.4%） and Yangming diseases（n=19,9.2%）.The cases of dampness-heat type（n=26） could be divided into 3 syndrome types including defense aspect syndrome（n=5）,qi aspect syndrome（n=2） and disease involving both defense aspect and qi aspect（n=19）.Conclusion The fever cases of acute tonsillitis can be inducted to 10 regular syndrome types,and the types of disease involving Taiyang,Shaoyang and Yangming,disease involving both Taiyang and Yangming and disease involving both Taiyang and Shaoyang are common.
