脂肪肝患者自恃心理调查及综合干预

Investigation on the self-reliance mentality of patients with fatty liver and comprehensive intervention

目的：探讨脂肪肝患者自恃心理程度及综合干预效果。方法：选择2009年1月-2010年6月来我院体检，查出符合中华医学会肝脏病学会脂肪肝和酒精性肝病学组制定的酒精性与非酒精性肝病诊断标准的脂肪肝患者120例为调查对象，采用自行设计的《脂肪肝患者自恃心理程度调查问卷》进行问卷调查，随机分为观察组和对照组各60例，对照组给予常规健康教育干预，观察组采取综合干预管理方法，建立以医护、患者、家属三者共同对疾病合作管理的模式。观察两组患者自恃心理程度、BMI、TG、肝脏B超结果有无差异。结果：脂肪肝患者自恃心理程度较重，积分均值是（1．88±0．11），积分较高，且普遍存在；两组患者自恃心理程度、BMI、TG、肝脏B超结果比较均有显著差异（P〈0．05）。结论：脂肪肝患者自恃心理的存在影响患者康复；综合干预能有效降低患者自恃心理程度，提高健康知识和自我管理水平；减少脂肪肝的恶化，改善生活质量。

Objective ： To explore the degree of self - reliance mentality of patients with fatty liver and the effect of comprehensive intervention. Methods： 120 fatty liver patients selected from those who had physical examination from January 2009 - June 2010 in our hospital were taken as the object of investigation and they were randomly divided into observation and control groups （ 60 cases for each group）. The patients who met the diagnostic criteria of fatty liver patients were selected according to the diagnostic criteria of alcoholic and non - alcoholic liver disease worked out by the liver disease branch of Chinese Medical Association. The self - designed questionnaire on the degree of self - reliance mentality of fatty liver patients was used to make an investigation. Routine health education intervention was taken in the control group ; the management method of comprehensive in- tervention was adopted in the observation group and a cooperative management model was established and consisted of doctors and nurses, patients and their family members. The degree of self - reliance mentality of fatty liver patients was compared between the two groups. BMI, TG and the diagnosis of liver by type B ultrasonic were not different. Resuhs ： The degree of self- reliance mentality of fatty liver patients was severe and the mean value was （ 1.88 ＋ 0.11 ）. The differences in the comparison of the de- gree of self - reliance mentality of the patients, BMI, TG and the diagnosis of liver by type B ultrasonic were significant between the two groups （ P 〈 0.05）. Conclusion ： The existence of self - reliance mentality affected the rehabilitation of patients with fatty liver. The comprehensive intervention can effectively reduce the degree of self - reliance mentality, increase health knowledge and self - management level, alleviate the deterioration of fatty liver and improve the quality of life of the patients with fatty liver.
