基本型间歇性外斜视术后远期正位率的影响因素分析

Influencing Factors of Long-term Orthotropia Rate after Primary Intermittent Exotropia Operation

目的分析基本型间歇性外斜视术后影响远期效果的因素。方法选择基本型间歇性外斜患者76例,男43例,女33例;年龄4～26岁,随访时间1～2年。观察患者年龄、术前三棱镜度数、视功能状态、手术方式、集合近点及术后早期的眼位等因素对术后远期正位率的影响。结果正位41例,欠矫26例,过矫9例。其中患者年龄、术前远立体视、术前集合近点及术后近期眼位对术后远期正位率的影响均有统计学意义(P<0.05);不同术前斜视度数和手术方式的术后远期正位率间差异无统计学意义(P>0.05)。结论早期手术、术前良好的视功能、正常的集合近点及术后早期的眼位正位,均有利于保持间歇性外斜视术后远期的正位率。

Objective To analyze the influencing factors of postoperative long-term orthotropia rate in patients with primary intermittent exotropia.Methods A total of 76 patients with intermittent exotropia（43 males,33 females,aged 4～26） underwent surgery and were followed up for 1~2 year.The effect of age,preoperative prism degree,visual functional status,operative models,near convergence point and postoperative early position of eyes were observed.Results Forty-one patients had normotopia,26 undercorrection,9 overcorrection.The effects of age,preoperative stereoacuity and near convergence point,postoperative short-term eye position on postoperative long-term orthotropia rate had significant difference（P0.05）,but no difference was noted between different preoperative strabismus degrees and different operative modes（P0.05）.Conclusion Early operative and preoperative good visual function,normal near convergence point,postoperative early normotopia of eye position are conducive to maintaining long-term normotopia rate after intermittent exotropia.
