胰腺实性假乳头状瘤的临床诊治（附25例报告）

Diagnosis and treatment of solid pseudopapillary tumor of the pancreas： A report of 25 cases

目的探讨胰腺实性假乳头状瘤（solid pseudopapmary tumor of the pancreas，SPTP）的诊断和治疗。方法回顾性分析2000年1月至2010年12月收治的经手术切除病理确诊的25例SPTP病人的临床资料。结果本组术前诊断SPTP2例。行胰腺体尾部切除11例，其中6例联合脾切除；行肿瘤切除6例；行胰腺节段切除4例；行胰十二指肠切除4例。术后发生胰漏3例，占全组病人的12％。病人术后恢复良好，全部均获随访，平均随访时间40．4个月。本组有3例分别于术后20个月、48个月及93个月发生肝转移，均经手术切除，术后再分别随访14个月，12个月，7个月，均无复发。结论SPTP是一种少见的多见于年轻女性的低度恶性胰腺肿瘤。术前确诊率低，手术切除是最有效的方法。

Objective To explore the diagnosis and treatment of solid pseudopapillary tumor of the pancreas（SPTP）. Methods From Jan. 2000 to Dec. 2（}10,25 cases of SPTP were reviewed in our hospital. Results Tumors were located in the pancreatic head and neck in 12 cases, and in the body and tail in 13 cases. The tumor in mean diameter was 5.5 cm（range 1.3-13. 0 cm）. All cases received surgical management, including the resection of pancreatic tail in 11 cases, of which 6 cases received splenectomy,resection of the tumors in 6 cases,partial pancreatectomy in 4 cases, pancreatoduodenec- tomy in 4 cases. All patients were followed up for the mean time of 40. 4 months（range 10-256 months）. Three cases had liver metastasis at the mouth 20,48,93 respectively after the resection, and the hepatic metastases were resected. The free-disease survival rate was 100%. Conclusion SPTP is a rare neoplasm of low malignant potential and is most common in young female. The preoperative diagnosis is difficult and the surgical resection may be the effect treatment.
